Every recommendation at Youthful Magnolia begins with a genuine conversation β about your biology, your goals, and what will actually move the needle for your skin long-term. No trending protocols. No shortcuts.
The Neck: The Most Overlooked Area in Facial Rejuvenation
Of all the areas patients bring to consultations at Youthful Magnolia in Fort Worth, the neck is the most consistently undertreated β and among the most visually impactful when addressed correctly.
The neck ages for reasons that are anatomically distinct from the face:
- The skin is thinner and has fewer sebaceous glands to support barrier function
- The platysma muscle plays a direct role in neck laxity and band formation
- Photodamage accumulates differently (often neglected in SPF routines)
- Volume loss doesn't affect the neck the way it does the mid-face
For these reasons, neck tightening requires a targeted, multi-layer strategy β not a repurposed facial protocol.
My Protocol for Non-Surgical Neck Tightening
At Youthful Magnolia, the approach I use most consistently for neck laxity is the same layered strategy that underlies my signature jowl lift protocol:
Ultherapy PRIME at 4.5mm (platysmal layer) + XERF multifrequency RF (dermal and subdermal layers)
This is also the core of the broader Ultherapy + XERF tightening approach I've developed for patients who want comprehensive, non-surgical results.
Why the Platysmal Layer Is Critical for the Neck
Ultherapy at 4.5mm targets the SMAS (Superficial Musculoaponeurotic System) in the face β but in the neck, the analogous structure is the platysma muscle and its surrounding fascia. This is the layer responsible for the "turkey neck" appearance β visible bands and general softening of the neck contour.
By delivering microfocused ultrasound energy at this depth, Ultherapy initiates collagen remodeling and structural tightening in the platysmal plane β producing gradual lifting and definition that develops over 2β6 months.
This is the depth that distinguishes Ultherapy from essentially every other non-surgical device available. No topical treatment, no surface-level RF, and no injectables can replicate structural tightening at this layer.

Where XERF Completes the Protocol
Once the structural foundation has been addressed by Ultherapy, XERF works through the dermal and subdermal layers above β improving skin firmness, collagen density, and the overall quality and "snap" of the neck skin.
Patients who undergo only Ultherapy often notice structural improvement but still have residual skin quality concerns. XERF addresses those upper-layer changes, completing the tightening effect across all meaningful tissue planes.
"The neck is one of the most honest indicators of age β and one of the most rewarding to treat. When you address the right layers in the right sequence, the improvement can be dramatic and completely natural-looking." β Amy Robbins

What About Neck Bands (Platysmal Bands)?
For patients with visible vertical platysmal bands, neuromodulator injections (Botox, Dysport, or Daxxify) into the platysma can soften the banding effect β a technique sometimes called a "Nefertiti lift" when combined with jawline treatment.
This injectable component is a valuable complement to energy-based tightening for patients with prominent bands. We assess this during the consultation and integrate it into the treatment plan when appropriate. See our injectables overview for more on neuromodulator options.
The Role of Photodamage in Neck Aging
Beyond laxity, many patients present with significant photodamage on the neck and dΓ©colletage β pigmentation, texture changes, and age spots that the face doesn't show as prominently (often because patients are more consistent with facial SPF).
For these concerns, BBL HERO extends the tone and pigmentation correction we apply to the face down through the neck β creating visual continuity and addressing the sun damage that accelerates the aged appearance of this area.
The combination of BBL for tone, Ultherapy for structure, and XERF for dermis gives the neck the same level of comprehensive attention most patients reserve for the face.
Adding Regenerative Support
For patients undergoing intensive neck tightening protocols, the results benefit from:
- PRF (Platelet-Rich Fibrin) β growth factor support for the collagen-building environment
- Peptide therapy β systemic regenerative support that extends result longevity
- IV therapy β cellular hydration and nutrient foundation during the treatment window
This reflects the regenerative medicine philosophy that distinguishes Youthful Magnolia's approach from standard medspa protocols.
Realistic Expectations for Non-Surgical Neck Tightening
I want to be honest about what non-surgical protocols can and cannot achieve for the neck:
Good candidates for non-surgical neck tightening:
- Early to moderate laxity (skin still has reasonable elasticity)
- Mild to moderate platysmal banding
- Photodamage and quality concerns without severe excess skin
- Patients who want to slow the progression of neck aging
- Patients in their late 30s to late 50s with maintained skin quality
Patients who may need a surgical conversation:
- Significant skin excess (more than 2β3cm of excess skin when pinched)
- Advanced platysmal banding that doesn't respond to neuromodulators
- Submental fat deposits that require liposuction
When non-surgical options are not sufficient, I will tell you directly and refer appropriately. This is how we build trust.
Treatment Planning and Sequencing
A typical neck tightening plan at Youthful Magnolia:
- Month 1: Ultherapy PRIME (full neck, including platysmal depth)
- Month 1β2: XERF series (can overlap or immediately follow)
- Concurrent: Peptide support during the collagen-building window
- Month 2β3: Neuromodulator injections if platysmal bands are present
- Month 3+: BBL if photodamage correction is a goal
- Maintenance: Annual or biennial Ultherapy; PRF support as needed
Frequently Asked Questions
How long does neck Ultherapy take? Neck Ultherapy typically takes 45β75 minutes depending on the area being treated. Combined face and neck sessions run longer.
Is neck Ultherapy painful? The neck can be one of the more sensitive treatment areas for Ultherapy. We apply topical numbing beforehand and pace the treatment to your comfort. XERF is significantly more comfortable.
How soon will I see neck tightening results? Some patients notice early tightening within 4β8 weeks. Most significant results develop over 3β6 months as collagen matures.
Can I combine neck tightening with facial treatments on the same day? Frequently yes β many patients treat the face and neck in the same Ultherapy session. We'll determine the right scope during your consultation.
How often should I repeat Ultherapy for the neck? Most patients benefit from annual Ultherapy to sustain and build on tightening results. XERF maintenance can be done more frequently.
